Patient-Centered Specialty Care

The Patient-Centered Specialty Care (PCSC) pilot program is designed to support specialty practices to align and coordinate care with their primary care counterparts. The current program includes physicians whose primary specialty is cardiology, endocrinology, and obstetrics/gynecology.

About the Program

Patient-Centered Specialty Care Program Pilot

The PCSC program adopts the principles of the American College of Physicians’ PCMH-N concept. It is designed to support specialty practices to align and coordinate care with their primary care counterparts. PCSC utilizes care compacts to help reduce duplicative testing, enhance access to care, improve care coordination and leverage integrated care planning. The ultimate goal is to foster effective health care teams to provide optimal patient care across the medical neighborhood.

Program Support

 The program’s primary support to participating practices is an online toolkit (below) that includes a self-paced, virtual learning curriculum, presentations, educational materials, templates and other resources.

PCSC Toolkit

The PCSC training program consists of viewing virtual learning collaborative sessions, in addition to submitting Plan-Do-Study-Act (PDSA) worksheets for three unique modules. Specialty care practices learn concepts through instructional webinars, practice these concepts using a PDSA cycle, and then reinforce the concepts by completing PDSA worksheets.
